Event Description

This course is designed to provide participants the required knowledge to understand the principle of incident investigations and the methods used to ensure an adequate investigation is completed. At the end of the session participants will have an understanding of the requirements for investigating incidents, the tools required to undertake an adequate investigation, methods involved in successful interviews, identifying incident causes and completing incident reports.

 
The program uses a classroom component with video and a multimedia PowerPoint presentation during the session. Participants will take part in a simulated investigation with witnesses to interview, a scene to evaluate and documentation to complete. This one day session is designed to provide participants with the tools to understand:
  • The regulatory requirements to investigate Accidents/Incidents
  • Why are we investigating Accidents/Incidents
  • Who makes a good investigator
  • How to gather evidence and information
  • How to photograph a scene
  • How to measure and diagram a scene
  • How to conduct interviews
  • What is the principle of Root Cause Analysis
  • Developing recommendations for prevention
  • Completing the Incident Report
Instructor: Terry Duncan is an independent safety specialist with over 30 years of experience in occupational safety. Past roles include safety coordinator for Dominion Bridge of Western Canada, and safety coordinator/industrial engineer for Freightliner Canada. Terry was also directly involved in the development of the City of Vancouver’s TSC/CSO training program. He has worked in construction, manufacturing, forestry, pulp and paper, and municipal and provincial governments. In the past Terry was voted Senior Safety Professional of the Year by the Canadian Society of Safety Engineering.

Cancellation &
Refund Policy

COURSE REGISTRATION CANCELLATIONS, TRANSFERS & REFUNDS:

 

Cancellations received at least 14 business days prior to the course will result in a full refund.

 

Cancellations received less than 14 business days but more than 7 full business days prior to the course are subject to an administration fee of $100 per day for full and half-day seminars, or $25 for breakfast sessions.

 

Registrants who do not show up for a course, and/or who do not notify CITN at least 7 full business days prior to the course date will be assessed the full cost of the course. There will be no refunds or credits in this case.

 

Substitution of attendees is permitted up to and including the day of the course.

 

COURSE CANCELLATION BY CITN:

 

CITN may have to postpone or cancel courses due to insufficient enrolment, in which case CITN’s liability will be limited to a refund of the registration fee.
